The Scanning-Ahead Principle

The #1 difference between good and poor sight-readers: eye-tempo separation. Good sight-readers' eyes are always 2-4 beats ahead of their hands. While playing beat 1, they are already scanning beat 3 or 4. This is trainable — practice reading one measure ahead while playing the current measure.
